This form is for providing medications or scripts for your pet’s eye conditions. It requires a 24-hour processing time and a review of the patient’s history by a veterinarian.
The clinic is an ophthalmology-only facility, and only dispenses repeat medications for eye conditions. Non-urgent scripts cost $15+GST, urgent scripts $25+GST, and scripts prepared outside normal business hours incur $40+GST.
The Pharmacy Guild of Australia advises that hard-copy scripts will be accepted. Cold medications (Cidofovir and Interferon in Tears) will not be posted. Cold medications can be picked up from East Malvern or other clinics.
A veterinarian will review your request. If approved a nurse will contact you by phone to let you know when it will be ready and to take payment.
- Charge for non urgent scripts (including postage of script) – $15+GST
- Charge for urgent scripts (less than 6 hours) – $25+GST
- Charge for scripts prepared out of normal hours – $40+GST
- Charge for actual medications (if supplied) – Please contact the clinic
- Charge for medication postage – $19+GST (legally scheduled medications must be trackable when posting)
